AT THE BEDSIDE In Our Journals Hot topics from this month’s AACN journal The Food and Drug Administration approved three intraosseous devices for use when intravenous access cannot be obtained in adult trauma patients. Insertion at the sternum, tibia or humerus generally takes less than one minute, and flow rates up to 125 mL/min can be achieved. Intended for emergency resuscitation, the devices should be removed within 24 hours after insertion, or as soon as practical after conventional peripheral or central intravenous access is established. Contraindications include fractures or other trauma at the insertion site, prosthetic joints near the site, previous attempts to insert an intraosseous device, osteoporosis or other bone abnormali- ties, infections at the proposed site and inability to identify pertinent insertion landmarks. (Day et al, CCN, Apr 2011) Emerging evidence suggests that nursing faculty and students also experience incivility in the educational and clinical environments. Nurses at the bedside should be concerned about incivility in academia, because they work intermittently with students and today’s students are tomorrow’s colleagues. The author identifies four areas for exploration: identifying where the propensity for incivility begins; where the emphasis on student learning and evaluation should be placed; how well staff nurses are prepared to participate in the educational process; and a critical examination of how students are socialized into the facebook.com/ccnface entirety of their role as future nurses. They are issues within the real world of nursing where a change in practice could improve patient care immediately. The issues are: Trendelenburg positioning for hypotension; use of rectal tubes to manage fecal incontinence; gastric residual volume and aspiration risk; restricted visiting policies; nursing interventions to reduce urinary catheter-associated infections; use of cell phones in critical care areas; and accurate assessment of body temperature. EOE High levels of high-density lipoprotein cholesterol (HDL-C) may be associated with a decreased risk of Alzheimer’s disease in elderly people, a recent study finds. “Association of Higher Levels of High-Density Lipoprotein Cholesterol in Elderly Individuals and Lower Risk of LateOnset Alzheimer Disease” in December’s Archives of Neurology shows that higher levels of HDL-C, greater than 55 mg/dL, decrease the risk of both probable and possible Alzheimer’s disease compared with lower levels. The study, which involved 1,130 elderly people with no cognitive impairment, also found that higher levels of total and non-HDL-C were associated with a decreased risk of Alzheimer’s when adjusted for age, sex, education, ethnic group and APOE e4 genotype. In a related HealthDay article, study author Christiane Reitz, assistant professor of neurology, Columbia University’s Taub Institute for Research on Alzheimer’s Disease and the Aging Brain, New York, says people can cut their risk by reducing intake of trans fats and increasing monounsaturated fats that keep “good” cholesterol high and “bad” cholesterol low; drinking moderate amounts of alcohol also helps.
