It turned out that this could be determined
with reasonable certainty in more than threequarters of them, including 86 percent of the
fatal accidents-and while transition training
damaged the greatest number of aircraft,
instrument instruction caused the most
deaths. Overall, it accounted for 11 percent of
all advanced training accidents but 21 percent
of the fatal accidents. In nearly half of all
IFR training flights that entered the National
Transportation Safety Board's (NTSB) case
files, someone on board got killed.
Stranger still is that it's not the instrument
flying itself that's getting people into trouble.
Just two fatal accidents were due to poor
execution of published procedures, and in one

of those a controller's failure to correct the
erroneous readback of an altitude restriction
also played a part. A third resulted from
a low-altitude stall while attempting an
unauthorized circling maneuver below MDA.
The aircraft was in VMC at that moment, but
only after passing the point at which both
regulations and common sense required a
missed approach. Only one accident was
attributed to an in-flight loss of control (due
in part to an inoperative turn coordinator); the
airframe was overstressed during the recovery,
but no one was hurt.
So if the problem's not controlling the airplane
by instrument references, what is it? The
answers are somewhat discouraging. The most

common causes of accidents on instrument
training flights were bad landings and
fuel mismanagement at 18 percent apiece.
To be fair, the combined proportion of
takeoff, landing, and go-around accidents
was only about half that in advanced
instruction overall-but the share due to
fuel mismanagement was more than three
times higher, and one-third were fatal. Is
it too much to hope that between them a
flight instructor and a certificated pilot
could manage to figure out how much gas
they needed, get at least that amount on
board, and see to it that it kept flowing
to the engine(s)? In almost half of those
accidents, usable fuel was still on board
when the airplane hit the ground.
