your APMA SUPPLEMENT TO APMA NEWS Trusted News and Information for Young Physicians APRIL 2012 / ISSUE 2 APPLYING FOR MEDICAL STAFF MEMBERSHIP AND HOSPITAL PRIVILEGES: A STEP-BY-STEP GUIDE FOR YOUNG PHYSICIANS APMA wants to help you as you prepare for a fulfilling career in podiatric medicine and surgery. To assist young physicians in applying for medical staff membership and hospital privileges, APMA has created the following step-by-step guide. U STEP 1: DO YOUR HOMEWORK nderstanding your hospital’s credentialing and privileging process, as well as the laws behind that process, is the first step to attaining privileges. The process is typically detailed in the medical staff bylaws or related policies and generally requires the following: 1) the practitioner requests an application; 2) the practitioner files a complete application with the medical staff; 3) the medical staff processes the application; 4) one or more medical staff committee(s) evaluate the practitioner’s credentials against the hospital’s requirements for membership and privileges; 5) the medical staff makes a recommendation to the governing body; and 6) the governing body takes final action on the application, either appointing the applicant to the medical staff with some or all of the requested privileges, or denying the applicant medical staff membership and privileges. The credentialing and privileging process is primarily governed by three main sources of law: 1) CMS Conditions of Participation for Hospitals (federal law); 2) The Joint Commission on Accreditation of Healthcare Organizations; and 3) state laws. To fully understand the application process, applicants should obtain and carefully review the following documents before applying for privileges: 1) an application, along with any explanatory documents; 2) the medical staff bylaws; 3) the medical staff rules and regulations; 4) the documents (such as rules and regulations or policies and procedures) directly governing the practice of podiatric medicine at the hospital; and 5) any other medical staff document that identifies the minimum qualifications necessary to practice podiatric medicine at the hospital. STEP 2: SUBMIT A COMPLETE APPLICATION Before granting privileges, hospitals must determine whether an applicant meets the criteria set forth by CMS and the Joint Commission. CMS requires an evaluation of character, competence, training, experience, and judgment. The Joint Commission requires hospitals to evaluate the applicant’s physical ability to perform the privilege, information from facilities where the applicant currently practices (if any), and peer and/or faculty recommendations. The applicant should provide the following to prove he or she meets those criteria: 1) evidence that he or she is licensed and graduated from an accredited podiatric medical college and an approved postgraduate training program; 2) reference letters or the names of references who are familiar with the practitioner’s clinical skills and competency; 3) case lists that confirm that he or she has Continued on Page 2 APMA IS SOCIAL: ARE YOU?
