Meaningful Use—What Stage Are We in Now?
CMS provides incentive payments for eligible providers (EP)
who demonstrate meaningful use of a certified electronic health
record (EHR) product. The program of incentive payments began in 2011, and podiatrists have been actively involved. In fact,
CMS statistics through December 2012 indicate 7,689 podiatrists have registered a certified EHR, and between 2011 and
2012, 3,934 unique podiatry providers have received a total of
$80,919,023 in payments.
The program is designed to have various stages of meaningful use. All providers must meet the requirements of stage
1 of meaningful use. In a nutshell, that means meeting 15 core
objectives and five out of 10 menu set objectives, and reporting on clinical quality measures (three core or alternate core
measures and then three out of 38 additional measures). In
2014, stage 2 of meaningful use will begin for those who began the process in either 2011 or 2012. Other providers will
remain in stage 1 or start in stage 1, if they are just beginning
the process. The following chart summarizes the stages of
meaningful use:

While stage 2 does not begin until 2014, the final rule that
established stage 2 of meaningful use also made some changes to stage 1. Most of those changes are optional in 2013,
meaning that the EP can either continue with stage 1 requirements as they were in 2012 and 2011 or make the changes in
2013. The choice is left up to the provider, but in 2014, the
requirements will no longer be optional.
Below is a summary of the changes to stage 1 of meaningful use (optional in 2013, required in 2014):
Computerized Physician Order Entry:
Beginning in 2013, CMS added an optional alternate measure to the objective for computerized
provider order entry (CPOE). The current measure

for CPOE is based on the number of unique patients with a medication in their medication list that
was entered using CPOE. The new, alternate measure is based on the total number of medication
orders created during the EHR reporting period.
Record and Chart Changes in Vital Signs:
CMS is changing the measure of the objective for
recording and charting changes in vital signs. The
current measure specifies that vital signs must be
recorded for more than 50 percent of all unique
patients ages two and older. The new measure
amends that age limit to recording blood pressure
for patients ages three and older and height and
weight for patients of all ages.
The exclusions for this objective for EPs are also
changing. The current exclusions only allow an
EP to claim the exclusion if all three vital signs
(height, weight, and blood pressure) are not relevant to their scope of practice, or if the EP sees no
patients two years of age or older. However, under
the new stage 1 changes, an EP can claim an exclusion if the EP sees no patients ages three years
or older (the EP would not have to record blood
pressure), if all three vital signs are not relevant to
his or her scope of practice (the EP would not record any vital signs), if height and weight are not
relevant to his or her scope of practice (the EP
would still record blood pressure), or if blood
pressure is not relevant to his or her scope of practice (the EP would still record height and weight).

Electronic Prescribing:
Beginning in 2013, CMS added an additional exclusion to the objective for electronic prescribing
for providers who are not within a 10-mile radius
of a pharmacy that accepts electronic prescriptions.

Electronic Exchange of Key Clinical Information:
Beginning in 2013, the objective for electronic exchange of key clinical information will no longer
be required for stage 1.
