MIPS: Beyond Avoiding the Penalty
Avoiding the penalty should be
the baseline for everyone, but
there are also incentive monies
available. If you don't investigate
this opportunity, you are potentially
leaving money on the table.
As the CMS Quality Payment Program (QPP) moves into its
second year, the majority of podiatrists will again be involved
with the Merit-based Incentive Payment System (MIPS) for
2018. It is anticipated that CMS will soon have an updated
2018 lookup tool on its website, www.qpp.cms.gov, where you
can enter your individual NPI number to see if you are required
to participate in the MIPS program for 2018. Remember, participation is based on any NPI/TIN combinations you bill under,
so you may be required to participate in MIPS for one combination but not for others if you bill under more than one TIN.
APMA has outlined strategies you can employ to avoid the
MIPS penalty based on 2018 performance-which will be a
5-percent reduction on all Medicare Part B payments in 2020.

you must achieve in 2018 to avoid the 2020 penalty is 15 (in
2017 it was a MIPS score of 3).
Avoiding the penalty should be the baseline for everyone,
but there are also incentive monies available. If you don't investigate this opportunity, you are potentially leaving money on the
table. As noted, the MIPS score you need to avoid the penalty
is 15, while the highest achievable MIPS score is 100. Incentive payments come from two sources. The money obtained
from eligible providers who are assessed a penalty is available to
provide positive payments to those with a MIPS score above 15.
An additional pool of money is available for "exceptional performers;" it is anticipated that you will need a MIPS score of 70
or above to be considered an "exceptional performer." The categories are broken down as shown by "Chart 1" at bottom left.
"Chart 2" at bottom right depicts categories available for groups
of 15 or fewer by applying to CMS.
The difference in the two charts is that providers in groups
of 15 or fewer can apply for an exception from the ACI component, and thus the Quality component would increase
