Benchmarking to improve
outcomes and reduce costs
PHIS comparative data helps hospitals identify areas of
improvement within specific populations and care practices.
A
sthma, pneumonia and bronchiolitis
are leading causes of admission for
pediatric patients. However, treatment
for these conditions varies greatly
among hospitals using care services
data from PHIS, part of CHA's Pediatric
Analytic Solutions portfolio. In 2012,
Pediatrics published a study that used
the achievable benchmarks of care (ABC)
methodology to identify potentially
better-performing hospitals in relation to
these conditions. The ABC methodology
uses existing data to calculate standards
$1.6 million
savings over three years
based on the top performers in a peer
group. In that study, Ann & Robert H.
Lurie Children's Hospital of Chicago
ranked 33 out of 42 hospitals across
the cumulative indicators for asthma,
bronchiolitis and pneumonia, indicating
the hospital had significant opportunities
for improvement.
Two years later, CHA completed a
reassessment of the rankings using 2014
data, which showed the hospital shifted
into second place out of 42. "Bronchiolitis
was a high-volume and high-variation
diagnosis," says Sangeeta Schroeder,
M.D., hospitalist at Lurie Children's. "In
2013, we put together a multidisciplinary
team to create a clinical care guideline
focused on reducing the use of
unnecessary testing and treatments, and
we saw areas we could improve on." The
bronchiolitis team at Lurie Children's
took a new approach to these common
conditions, which resulted in positive
outcomes for patients and the hospital.
Results included more appropriately
placing inpatient and observation
patients with bronchiolitis.
Administration of more than one dose
of a bronchodilator decreased from
42 percent to 11 percent, which is in
accordance with the peer benchmark. And
chest X-ray use stabilized at 20 percent,
well below the norm for other hospitals.
By reducing variation in care, the hospital
also reduced costs per patient by 33
percent. This translated to savings of $1.6
million-or almost $5,000 per patient-
between June 2012 and March 2015.

the Children's Hospital Association's Speak
Now for Kids Family Advocacy Day, June 22 in
Washington, D.C. They discussed with legislators
how passing the ACE Kids Act would help improve
quality and care coordination for children with
medical complexity.
