CONTEST RULES 1. How to Enter Contests. Online: To enter a contest announced in FREE LAUNCH in the November 2011 issue, log on to BeautyLaunchpad.com and follow the instructions for contest entries. Online: entries must be received no later than November 30, 2011. By mail: Send a 3 1/2 x 5-inch postcard, stating your name, mailing address, telephone number, email address and the contest you wish to enter, to FREE LAUNCH, Beauty Launchpad Magazine, 7628 Densmore Avenue, Van Nuys, CA 91406-2042. Postcards must be postmarked no later than November 30, 2011. 2. Who May Enter? Contests are open to legal U.S. residents 18 years of age or older who are (a) licensed cosmetologists, (b) other professionals currently employed in the beauty industry, and/or (c) owners and operators of salons where licensed cosmetologists and other beauty industry professionals are employed. Employees of Creative Age Publications, Inc., and their family members; employees and family members of participating advertisers, their affiliates and their advertising, promotional or other agents do not qualify to enter. 3. Limits on Entries. Each contest will be conducted separately and must be entered separately. Entries in each contest are limited to one entry per person, per day regardless of method of entry. Entries by email are limited to one entry per email address, per day. 4. No Purchase Is Necessary. No purchase is necessary to enter or win any of the contests announced in FREE LAUNCH. A purchase or payment or any kind will not improve your chances of winning. 5. Selection of Winners. For each contest, participating advertisers will select the winning entries at random from among all qualified and timely entries no later than December 28, 2011, and will notify winners by phone by January 11, 2012. 6. Prizes. The number and description of prizes to be awarded in each contest will be determined by the participating advertiser and announced in FREE LAUNCH and/or at the FREE LAUNCH web site. All announced prizes will be awarded. Prizes may not be transferred or exchanged for cash. Participating advertisers reserve the right to substitute prizes of equal value. Odds of winning a prize depend on the number of entries in each contest. 7. Who Won? For the names of winners, send a self-addressed, stamped envelope after December 28, 2011 to FREE LAUNCH, Beauty Launchpad Magazine, 7628 Densmore Avenue, Van Nuys, CA 91406-2042. 8. General Rules. By entering a contest, entrants agree to be bound by all rules of each such contest, and further specifically agree as follows: Creative Age, the participating advertisers, and their agents, licensees, successors and assigns shall have the right, without compensation, to use the name, address, and other information provided by all entrants for advertising, publicity and promotional purposes, and the right to use the name, voice, image, likeness and biographical data of all winners for publicity, advertising and promotional purposes; and entrant waives, releases and holds harmless Creative Age, participating advertisers, and their agents, licensees, successors and assigns from and against any liability of any kind resulting from or relating to the contests, the prizes and/or any such advertising, publicity and promotion. To receive a prize, each winner may be required to sign and return an Affidavit of Eligibility and Release confirming eligibility and compliance with contest rules. 9. Void Where Prohibited. All contests are void where prohibited by law and subject to all local, state and federal laws, rules and regulations. Other restrictions may apply. 10. Participating Advertisers. Contests are sponsored and administered solely by the participating advertisers, who are solely responsible and liable for the contests that they sponsor. 11. Disclaimer. Creative Age and the participating advertisers disclaim any responsibility or liability for late, lost or illegible entries, or computer system, phone line, hardware, software or program malfunctions, or other errors, failures or delays in computer transmission, network connections, or postal delivery, whether human or technical in nature. 12.
