The average cost of bringing a new drug to market is $1.8 billion1, an amount of money that
would buy two trips to the moon, the Solomon islands, or 16 million official NFL footballs 2.
Apart from the cost, the development of a new molecular entities (NME) from initial screen to
the launch of a finished product can take 12-15 years with fewer than 1 in 10 drugs tested in
human clinical trials being successful1 (in cancer this attrition rate is as high as 1 in 20)3. Drugs
fail in the clinic for two main reasons: the first is that they fail to show efficacy in vivo and the
second is that they show unexpected toxic side effects. Late stage attrition is particularly costly
and might be avoided with better preclinical strategies to evaluate novel agents. This article
discusses why phenotypic screens of 3D cell cultures could deliver more predictive in vivo data
and explains how they can be adapted for high content screening.
