Laser-assisted endodontic treatment is an important new innovation that has the potential to signiﬁcantly improve endodontic outcomes. Laser-Assisted Endodontic Treatment Is this tooth really worth keeping? That is the question being asked on a daily basis by clinicians, as they weigh the relative merits of treating a tooth with “issues,” as opposed to replacing it with an implant. In endodontics, there are no “rules of thumb” as to how these questions are answered. This essay provides some insight into the treatment planning of such cases, with the ultimate goal of providing the best possible service for the patient. An Inconvenient Tooth Pain represents patients’ primary concern about endodontic treatment. Today, endodontic therapy emphasizes preventive strategies rather than reacting to pain after it occurs. This essay reviews speciﬁc strategies that will enable clinicians to provide endodontic therapy as a predictably painless procedure. The technician takes a two dimensional concept and turns it into a three-dimensional model. All of the steps that are critical to success are discussed, as well as, the tools for achieving successful communication between the specialist, the dentist and the technician in an effort to satisfy the patient. The planning and execution must be done in complete harmony with the goals of all three team members. The differences in protocols for various types of restorative situations are also reviewed. The Laboratory Role in Achieving a Predictable Aesthetic Result Cosmetic dentistry encompasses a disciplined approach to patient care. The role of the restorative dentist is to coordinate both periodontal and laboratory procedures. Most cosmetic cases require periodontal corrections of either crown lengthening or root coverage for proportional tooth symmetry. The lab technician plays an integral part in the smile design thru diagnostic wax ups, preparation guides and gum lift guides. Once all these variables are addressed the dentist can develop an approach of “begin with the end in mind”. It is therefore, the restorative dentists’ responsibility to develop the esthetic plan using a multidisciplinary approach. Multidisciplinary Esthetic Principles An ideal periodontal foundation sets the background or “matting/framing” of the ﬁnal aesthetic result. Alone, the soft tissue proﬁle is important for health, but properly coordinated with the ceramist and restorative dentist, the aesthetic outcome can be both natural and beautiful. This takes constant communication and knowledge by the team. A prosthetic guide for this result requires not only a porcelain guide but a periodontal guide as to gingival symmetry, papillas, tooth length, biotype, and bone to name a few parameters.
