The protocol for placing MDIs is directly through gingival tissues, via ﬂapless surgery. The goal is to complete a comprehensive examination that allows the creation of a stable balanced occlusion. The occlusion should support the TMJ joints in a most stable and repeatable position. Sometimes this restorative position is achieved through increasing the vertical dimension of occlusion. The use of appliances (maxillary and mandibular) as well as provisionals and even direct restorative materials to create a trial reconstruction are presented.
