HQP continued our thought leadership with a
presentation on behavioral health integration for the
NACHC's PCA/HCCN conference in Pasadena, CA. 

INNOVATION & THOUGHT LEADERSHIP 
GOAL: Position primary care as fundamental to the
health care landscape in the U.S. 

HQP staff co-produced the Annual Healthcare
Symposium in conjunction with the Community
Clinic Association of Los Angeles County and the
Coalition of Orange County Community Health
Centers, which drew more than 400 attendees. HCP/
HQP staff and or member centers' staff presented in
seven of the fifteen sessions and moderated five. 

In August 2016, HQP began working with the National
Association of Community Health Centers (NACHC) and
other California consortia on a grant proposal to Blue
Shield of California Foundation to develop a roadmap to
bring the PRAPARE model to scale in California. PRAPARE
(Protocol for Responding to and Assessing Patients'
Assets, Risks, and Experiences) is a national effort to help
health centers collect data to better understand and
address social determinants of health. By January 2017,
HQP engaged two of its member health centers to
participate. We are one of only four consortia
participating in this planning project with foundation
funding. 

CLINICAL CARE COORDINATION &
QUALITY IMPROVEMENT
In keeping with its long-range plan to become an
innovation hub and incubator to improve primary
care, HQP continued to promote and field-test new
programs and services, 

In September, HQP participated in a southern California
Kaiser Permanente Community Benefit meeting with their
National Program Office leaders to discuss current work
and future opportunities for strengthening the region's
safety net.
