Integrated Health Partners of Southern California
is a network that unites community health centers
for the common purpose of advancing standards
of quality care, improving health outcomes and
reducing costs.
Phase 3, the learning series included the introduction
of tools, testing and optimization of population health
management strategies with bi-weekly webinars
focused on population health management strategies.
Tools, implementation strategies, and outcome
objectives were distributed including:  scheduling/
access; IT infrastructure; data needs; quality
improvement; and, care coordination / risk stratification.
Once distributed, collaborative learning time was
reserved to identify best practices, problems and
resolutions with the goal to sustain monitoring of
population health management strategies, and
measuring their impact on Clinically Integrated Network
(CIN) outcomes.

INNOVATION & THOUGHT LEADERSHIP 
GOAL:  Position primary care as fundamental to the
health care landscape in the U.S. 
IHP stepped up its thought leadership in FY16-17, with
participation in key industry conferences and meetings
with other Primary Care Associations, including using
non-traditional business strategies to drive a mission at
the CPCA Annual Conference, and building relationships
with payers at the NACHC PCA/HCCN conference.
In November 2016, IHP hosted Adam Falcone, partner at
Feldesman, Tucker, Leifer, Fidell, in a first step toward an
in-depth review of its integration performance. 

Phase 4 concluded with dedicated transformation
work with health centers based on risk/need identified
within the assessment phase with one-on-one
transformation work tailored to fit health centers'
needs. IHP held an all-hands retreat, facilitated by
change management consultant, Peter Stark, involving
Adam Falcone and Centerprise's VP Shannon
Nielson.Discussion focused on articulating the vision
and the administrative, clinical, financial and IT
strategies to support clinical integration for the coming
year.  The retreat was supported in part by a grant from
the California Health Care Foundation.

GOAL: Facilitate capacity building and increase
readiness for tighter clinical and financial integration.
Work continued with Centerprise to position IHP for
success and to optimize health center operations. This
work broke down into four phases: (1) pre-assessment;
(2) assessment/launch; (3) learning series; and,
(4) health center transformation. Phases 1 and 2 were
completed in FY 15/16.
