Decentralized nurses' stations yield
surprising results
Researchers at Ann & Robert H. Lurie Children's Hospital
of Chicago studied the effects of switching from a central
nurses' station in its old facility to decentralized stations
in its new building, which opened two years ago.
While many of the study's expectations were met,
there were some surprises, shared Sherri Ewing, associate chief nurse executive; Karen Richey, administrator of
acute care services; and nurse Huong Mai, all from Lurie
Children's, during a session at the Healthcare Design
Conference.
Staff transitioned from a racetrack layout with a
centralized nurses' station and two smaller workstations to a unit that uses individual workstations
("porches") throughout to locate staff closer to patients.
"Consultation zones" outside patient rooms and "care
team stations"-multiuse work spaces with both open
and private areas-were included in the new space, as
well.
Anticipated benefits of the new layout included improved communication and collaboration among care
team members, better efficiency in service delivery, more
privacy, and reduced noise levels. But the findings don't
paint quite that rosy a picture.
Staff reported less satisfaction with their jobs in the
new setup and a weakened sense of teamwork. They
found workflow to be less efficient and face-to-face communication has decreased.
Researchers suggested that these kinds of findings
may be due to the increased size of the unit, the fact that
the new hospital consolidated what had previously been
two units into one, and the feeling of increased isolation
that comes naturally with a switch to individual workstations. On the positive side, the perception of noise
was decreased, as was the number of distractions for
workers.
One finding that caught attendees' attention was that
nurses in the new unit walked half a mile more than they
had in the previous space despite the new arrangement.
The researchers didn't offer a direct explanation for
this, but they did shed some light on the culture in the
new space, including the fact that caregivers tended to
gravitate toward the bigger care team stations to (in part)
reclaim the camaraderie and support they were missing.

right balance between staff needs and improved access
