Chapter 8. Passenger Special Plates Issued by Branch: Medal of Liberty Plate continued: E. Approval Process: An Application for Medal of Liberty plates must be completed. The applicants must first be approved for the Massachusetts Medal of Liberty by the Office of the Adjutant General. Once approved for the medal, they can either apply for the plates online or by mail. All of the following documents must be submitted: 1. A completed Application for Medal of Liberty Plates 2. A Recommendation for the Massachusetts Medal of Liberty Form (TAGMA form 3367) approved by the office of the Adjutant General. 3. The service member's DD-2214, DD From 1300, Deployment Orders, or NGB22. 4. A completed Registration and Title Application (stamped by an insurance company) Once approved for plates, they will be mailed to the applicants and should be received within 20 days, at which point they can be attached to the vehicle. Killed in Action (KIA) Decals Applicants can request a KIA decal if the service member's DD Form 1300 indicated " Killed in Action " in category box 4C. K.I.A.
