T

he number of women of all ages having
hysterectomies is on the rise. In fact,
according to the U.S. Department
of Health and Human Services,
hysterectomy is the second most common
surgery for women in the U.S., with the first
being cesarean deliveries during childbirth.
Here's a guide to hysterectomies, why this
surgery may be needed and what to expect.

What is a
hysterectomy?

A hysterectomy is the surgical
removal of the uterus, also known
as the womb. This is the muscular,
hollow organ that holds a baby as
it grows. In addition to the uterus,
a woman's reproductive system
consists of the vagina, ovaries and
fallopian tubes.
Having a hysterectomy means
a woman will no longer have
periods or become pregnant.
If the ovaries-the hormone
producers of the reproductive
system-are also removed,
menopause begins immediately.
While surgical techniques today
are often minimally invasive, a
hysterectomy will require two to
eight weeks recovery time.

Why might a
hysterectomy
be needed?

A wide variety of benign
(noncancerous) conditions
can cause chronic pain and
heavy bleeding as well as other
symptoms. Some of these
conditions include:

What are the
different types of
hysterectomies?

Nathan Mordel, MD

3 Fibroids-one or more
noncancerous growths in the
uterine wall.
3 Endometriosis-a painful
condition in which tissue that
normally lines the inside of the
uterus grows outside the uterus.
3 Uterine prolapse-when
the uterus falls or slips into
the vagina.
In addition, if a woman has
endometrial, uterine or other
cancers in the pelvic region,
a hysterectomy may be needed.
Symptoms that warrant
consultation with your healthcare
provider may include any of
the following:
* heavy, long or painful
menstrual periods
* painful cramping
* pelvic pain or pressure
* painful intercourse
* difficulties with bowel or
bladder control
* infertility
* backache or leg pain
* a sensation of pulling or
heaviness inside the pelvis
* tissue protruding from the
vagina

The type of hysterectomy needed
depends on many factors. A woman
facing hysterectomy should make
sure she understands exactly what
is being recommended and why,
and what she can expect long-term
after the surgery.
There are three types
of hysterectomy:
3 Supracervical hysterectomy
removes the uterus but not
the cervix.
3 Total hysterectomy removes
both the uterus and cervix.
3 Radical (or modified
radical) hysterectomy is a
more extensive surgery, usually
for cases of gynecologic cancer.
This surgery removes the
uterus, cervix, and possibly
more, depending on how far
the cancer has spread.

What are the
alternatives?

For fibroids, endometriosis,
uterine prolapse or other pelvic conditions, generally the
first choice of treatment will be
watchful waiting or treatment of
the symptoms with medication.
Other options to deal with
fibroids may include: myomectomy, surgical removal of only
the fibroids; uterine fibroid embolization, also known as uterine
artery embolization, which cuts
off the fibroid's blood supply; or
hysteroscopy, a minor surgical
procedure to remove fibroids that
are inside the uterus.
