educating the patient on safe opioid use, and communicating
the discharge plan with the outpatient provider.14,15 TJC
recently revised pain assessment and management standards
for hospitals to include safe opioid prescribing as an organizational priority starting in 2018.16
At Fairview Health Services, support has been growing to
include pharmacists as members of interdisciplinary teams to
address complex medication management. The inpatient
pain management specialist role for pharmacists has evolved
over several years at UMMC for collaborative activities with
anesthesia services on the acute pain team. Pain team pharmacists recommend preoperative medication therapy plans,
provide inpatient pain management consults, and develop
pain medication discharge transition plans.
As the opioid stewardship model progresses down the
long road, emphasis will need to be placed on activities to
improve disparities that exist for patients diagnosed with
opioid substance use disorder. Opioid stewardship for this
group of patients requires collaboration with other services
including psychiatry, chemical dependency, and social
work. The expansion of opioid stewardship should address
the care transition of patients on methadone or buprenorphine for medication-assisted treatment (MAT). As an
example, pharmacists can help bridge communication gaps
at discharge through patient education and documentation

of the monitoring plan with the clinic medication therapy
management pharmacist. Exploring new opportunities for
MAT should be part of the continuous process improvement
for opioid stewardship.

Institutionalize New Approaches
Leadership has an important role in embedding the opioid
stewardship model as a conventional norm in the patient
safety culture throughout the organization. To accomplish
this, frequent communication from organization leaders is
needed on how the new changes have improved performance. Tracking performance metrics on processes that have
the greatest impact on the organization is fundamental to opioid stewardship. In a Lean hospital environment, the processes include safety, quality, delivery, cost, and morale
components of satisfaction with both patients and health care
providers.7
The balanced scorecard is a tool that focuses on performance measures for strategy and vision to provide leaders
with a fast, comprehensive view of the organization.
Compliance goals are displayed on the scorecard for the
behaviors and actions in a format that uses easy-to-read
charts or graphs. The measures are designed to pull people
toward the overall vision.17
