A Multi-institutional Survey Study to Assess Medical
Student Experiences on Otolaryngology-Head and
Neck Surgery Clerkships

Objectives: Factors that influence medical student career
selection include access to positive role models and early academic opportunities. To date, there is limited research on the
factors that may influence medical students to pursue otolaryngology-head and neck surgery (OHNS). The purpose of
this study was to explore the impact of an OHNS rotation on
medical student perceptions of OHNS.
Methods: In September 2019, an electronic survey link was
sent to medical students at 5 academic institutions who completed an OHNS clinical rotation (n = 218). The survey consisted of 23 questions, including Likert-style questions (range,
1-5). Statistical analysis included paired t tests.
Results: A total of 64 medical students completed the survey
(response rate = 29.4%), of whom 92% were in their fourth
year and 64% were men. There was an increased interest in
OHNS after clerkship completion (2.1 vs 3.0, P < .0001), confidence with the head and neck exam (2.1 vs 3.5, P < .0001),
and confidence in examining patients with tracheostomy tubes
(1.2 vs 2.8, P < .0001). Among participants, 67.2% stated their
clerkship was sufficient to consider OHNS as a career. Faculty
interaction (73.4%) and resident interaction (67.2%) were
rated the most influential factors in considering OHNS as a
career. Medical students reported USMLE Step Examination
scores (95.3%) and research experience (87.5%) to be the
most important factors when matching into OHNS.
Conclusions: OHNS clerkships can be valuable experiences
for medical students to gain head and neck examination skills.
There was an increased interest in OHNS as a career after
completing the rotation. Core surgery clerkship directors
should consider expanding opportunities for medical students
to gain exposure to OHNS.

Resident Curriculum: The Needs Assessment Process

Objectives: The 2016 modifications to the Accreditation
Council for Graduate Medical Education Otolaryngology postgraduate year (PGY)-1 program requirements resulted in a
wide variation of experiences, evaluation methodologies, and
perceived readiness for PGY-2. The Otolaryngology Program
Directors Organization approved a task force to study the issue
and to create a standardized curriculum for PGY-1 trainees.
Methods: A needs assessment was performed using a modified Delphi approach. A series of iterative surveys were developed to obtain broad opinion on the necessary knowledge,
skills, and behaviors for an intern curriculum. The anonymous
surveys were distributed to a diverse group of stakeholders,
including trainees, program directors, faculty members, allied

health professionals, nursing, and academic chairs between
March 2019 and June 2019. An initial list of topics was generated, collated, and ranked in order of importance for inclusion
in the curriculum.
Results: Fifty-three responders identified 1009 unique topics
for inclusion in a standardized PGY-1 curriculum. These were
codified by 2 reviewers into 120 categories across general and
otolaryngology-specific knowledge, skills, and attitudes. The 10
highest priority topics were further developed by task force members into formal goals and objectives as a pilot demonstration.
Conclusions: Needs assessment is an important stage of curriculum development. The iterative and anonymous nature of
a modified Delphi approach allows for a wide range of expert
stakeholder opinions. This strategy applied to curriculum
development allows for formation of a curated otolaryngology
curriculum for PGY-1 residents.
Nononcologic Uses of Transoral Robotic Surgery

Objectives: Transoral robotic surgery (TORS) is most commonly used for oropharyngeal and laryngeal squamous cell
carcinomas. The breadth of disorders and types of surgeries
for which this technology can be potentially used is evolving.
In this study, we describe outcomes of patients treated for nononcologic pathologies who underwent surgical management
with TORS at our institution.
Methods: A retrospective review from April 2010 until
March 2019 was performed on patients who underwent TORS
procedures for nononcologic diagnoses at our institution.
Results: Twenty-four patients were identified. Twelve
patients received TORS lingual tonsillectomy for lingual tonsillar hypertrophy. Two of these 12 patients also underwent
uvulopalatopharyngoplasty for sleep apnea, and 2 others
underwent a base-of-tongue reduction for sleep apnea. Six
patients underwent TORS styloid reduction for Eagle's syndrome. Two patients had parapharyngeal mass excisions, and
2 had retropharyngeal mass excisions. One patient underwent
base-of-tongue resection for a benign neuroma. One patient
had a tonsillar papilloma removed.
Conclusions: No surgical complications or postoperative
complications were encountered in any of the 24 patients.
Five of 6 patients treated for Eagle's syndrome reported resolution of pain postoperatively. Two of the 4 patients treated for
sleep apnea were seen postoperatively and demonstrated
markedly improved sleep studies.
