" Yes, there was a lower adherence
for Black patients, but when you look
closely, it is really related to finances.
When you look at those who had a
high net worth, there was no difference.
... And so it was the financial
implications for therapy that made a
difference in terms of compliance. "
REGARDING SOCIAL DETERMINANTS
OF HEALTH IN CLINICAL TRIALS...
" Look at economic stability, neighborhood
and physical environment,
education, food, community and social
context, and healthcare system-they
will dramatically impact the outcomes
for our patients. And we have to systematically
collect these data-we don't
have a good way of doing that, including
for clinical trials. "
REGARDING THE DEVELOPMENT OF
DISTANT RECURRENCE AFTER NEOADJUVANT
CHEMOTHERAPY SEEN IN
THE TMEM DOORWAY STUDY...
" While I think these data are very
provocative, I certainly wouldn't want
Black women or any women who need
neoadjuvant therapy to be discouraged
because of these data. We need these
data to be rigorously tested clinically.
REGARDING ANALYZING OUTCOMES
BY RACE IN CLINICAL TRIALS...
" We have to consider that there is
immense heterogeneity within races,
but we also have to think about the
arbitrariness of race. Quite simply, race
is a social construct [that has historically]
been based on power and greed. So
with that backdrop, how should we, as
researchers, analyze outcomes by race?
I think it is very important to consider
race, but we have to do so very thoughtfully
so that we don't perpetuate some
of the concepts that we have long tried
to disprove. Without a doubt, biology
and ancestry are so very important,
but we also have to try to dig deeper
to address the hidden factors-social
determinants of health-that contribute
to disparities attributed to race. I think
this is our collective responsibility, and I
believe the onus is on us to do so. "

We've seen other work published this year looking at potentially
other differences in the tumor immune microenvironment that exist
between people of different ethnicity or racial background. Have you
looked beyond the TMEM doorway at the tumor microenvironment in
residual tumor and are there any other interesting changes there?
Dr. Karadal: There is a part of
this study that was not included
in the presentation involving
cancer stem cells. We know from
published studies that TMEM
doorways, especially transmitting
macrophages and tumor cells,
[increase tumor cell activity], but
we didn't see any racial disparity
regarding this.

Black women with breast cancer are more likely to be diagnosed with advanced-stage disease,
have lower access to care, and have triple-negative disease, all of which contribute to higher
mortality rates compared with white women. The results from our study demonstrate differences
in the tumor microenvironment of Black women that may partly explain the racial disparities in the
outcomes of ER-positive/HER2-negative breast cancer, the most common breast cancer subtype in
both white and Black women. "
