Search: Way Off Base t New Members' Thoughts on Self-ligation I'm relatively new to the message boards, but I think this a great resource for educated discussion. One thing that has really bothered me is the debates over self-ligating brackets on this forum (and in the overall ortho community). A little background on me: I currently use self-ligating brackets. I took over a practice that was mainly using SmartClip, but also had In-Ovation and Damon. I have since converted almost exclusively to In-Ovation. I graduated in '08, so I realize I'm no expert on the subject yet. What I've seen in a few postings now is that when a case is discussed that involves a debate between extraction and non-extraction treatment plans, using self-ligating brackets is considered a treatment option. I find this a little troubling that orthodontists out there believe that self-ligating brackets will make the difference is these cases. In my opinion there is really only three ways to resolve crowding: extract, IPR or arch development (some combination of expanding and proclining). In getting arch development you either do it with additional appliances (i.e., RPE, TPA, etc.) or with braces alone. So far the literature doesn't support the idea that low-friction brackets move teeth any faster or more efficiently, although more research is still needed. So once again the idea that switching from an open slot to self-ligating for a case will somehow resolve more crowding seems crazy. How would the teeth know the difference between a self-ligating bracket and an open bracket loosely tied with a steel ligature? As I said, I use self-ligation and I think it has some benefits over traditional brackets, but when deciding what bracket to use is integral to the treatment planning process, it bothers me. I've attended a few of its CE, and I kid you not, they tell you in some cases to take a 19x25 SS and expand it so it is twice the width of the arch! (By the way, I am in no way advocating this.) Would that work in a non-Damon bracket? Probably not, because the wire would probably rip the ligature ties off. P.S. I do use Damon brackets in my practice. I like some aspects of it and hate some aspects of it. Learned some good tidbits at the CE, and learned some BS as well. Really cool. Wanted Damon Q because he read online and heard from the other guy in town that it would be faster and involved no extractions. He was the most crowded adult I have seen in years. I said 4 bi 2430 months. I sat with him for 20 minutes explaining my plight and I guess he liked what he heard because he got records. Funny thing, this guy is an engineer - he basically said something to the effect of (paraphrasing), "so in my circumstances pulling four teeth is potentially more conservative then squeezing everything in." When he asked why I would still use SLs on him. I said the adjustments are faster.
