Discussion
The goal of the informatics changes implemented at this
institution was to increase compliance to the specified REMS
requirements, improving overall patient safety. By updating
the ordering panel and including required questions, compliance to pregnancy tests being completed in FCBP and LFTs
performed in patients on bosentan increased to 100%.
Therefore, it is clear that this newly initiated informatics
update substantially improved overall compliance to the
REMS programs for the ERAs and riociguat.
Prior to the informatics update in March 2016, pharmacists were not prompted to create an iVent within the patient
chart to verify and document REMS requirements were met
by the ordering provider. With the informatics update,

pharmacists are alerted to complete the verification step,
ensuring all REMS requirements are met prior to verifying
the medication. This, along with other ordering changes, has
improved overall compliance with both pregnancy tests and
LFTs being performed within 30 days of medication
initiation.
CDS tools have been developed for other drugs that have
REMS at our institution, including clozapine, olanzapine
pamoate, vigabatrin, and buprenorphine/naloxone. However,
this is the first retrospective analysis of these tools' ability to
improve compliance. Due to the positive results, the implementation of new tools and optimization of current tools are being
assessed. In addition, retrospective analyses will be performed
on the other medications as more data becomes available and
potential surveys for feedback on the tools may be collected.
