Introduction: Remote Ischaemic Per-Conditioning (Ric) In Experimental
Ischaemic Stroke Is Cytoprotective And May Reduce Ischaemic
Reperfusion Injury. It Is Achieved By Repeated Transient Occlusion Of
The Blood Supply To A Limb Using A Blood Pressure Cuff. We Aim To
Assess Ric In Patients With Hyper-Acute Ischaemic Stroke.
Method: Recast-3 Is A Prospective, Randomised, Controlled, BlindedEndpoint,
Parallel-Group, Multicentre Trial. 1300 Adults With Hyperacute
Ischaemic Stroke (<6 Hours Post Onset, Nihss =4) Presenting To Stroke
Units In The Uk Will Be Recruited. Exclusion Criteria Include; Pre-Morbid
Dependency, Spontaneous Intracerebral Haemorrhage, Dementia, Coma,
Malignancy, Significant Co-Morbidity, Bm <3.0Mmol/L And Seizure On
Presentation Unless Brain Imaging Identifies Significant Brain Ischaemia.
Participants Will Be Randomised (1:1) To Receive Either Ric (4 Cycles
Alternating Between 5 Minutes Inflation To 25Mmhg Above Systolic Bp
And 5 Minutes Deflation) Or Sham. A Second Dose Will Be
Administered 1-2 Hours Later. Two Further Doses Are Given On The
Following Day.
Results: The Primary Outcome Is Death Or Dependency (Ordinal Shift
In The Modified Rankin Scale) At Day 90 Recorded Using Central Blinded
Telephone Follow-Up. Secondary Outcomes Include Cerebrovascular
Events, Major Adverse Cardiac And Cerebral Events, Acute Kidney Injury,
Disability, Cognition, Mood, Frailty, Quality Of Life, And Safety.
Conclusion: Recast-3 Will Provide Evidence For The Use Of Ric In
Hyperacute Stroke. The Results Will Inform On The Safety And
Improvement Of The Functional Outcome In Patients For Standardised
Practice In Hyper-Acute Stroke Treatment.

Of Life. Current Studies Suggest That Music-Based Games Encourage
High Repetitive Upper Limb Movement Using Its Music Component As
An Intrinsic Motivator. However, Evidence-Based Recommendations To
Guide Practice Are Lacking. This Review Aimed To Determine The
Effectiveness, Feasibility And Experience Of Using Music-Based Games
For Improving Upper Limb Function Post-Stroke.
Method: Eleven Databases Were Searched For Studies Of Any Design.
Two Independent Reviewers Selected Studies, Extracted Data, And
Evaluated Study Quality Using Published Tools. Random-Effect MetaAnalysis
Was Used To Determine The Effect Of Music-Based Games On
Upper Limb Function Outcomes. A Parallel-Results Convergent Synthesis
Design Was Used, Synthesising
Qualitative And Quantitative Data
Following Their Separate Analyses.
Results: Of 15, 456 Records, 10 Studies (Three Randomised Trials,
Three Cohorts, One Within-Subject Design, One Single-Case Design
And Two Qualitative) Involving 114 Stroke Survivors Met Inclusion
Criteria. Most Studies Were Of Moderate Quality. Interventions
Comprised Serious Games (5 Studies) And Commercial Games (5
Studies). Arm Capacity Measured Using The Action Research Arm Test
[Md: 1.47, 95% Ci (-0.98, 3.93), I2 =0%, P=0.24] And Dexterity Measured
Using The Box And Block Test [Md: 1.48, 95% Ci (-5.38, 8.34), I2 =69%,
P=0.67] Did Not Differ Between Intervention And Control Groups.
There Were No Intervention-Related Adverse Effects, And Dropout Rate
Ranged From 7 To 10%. User Experience Was Positive And Stroke
Survivors Enjoyed The Intervention.
Conclusion: Findings Suggest The Safety And Feasibility Of Music-Based
Games, But Are Not Superior To Conventional Rehabilitation In
Improving Upper Limb Function Post-Stroke.

(Pssuq). Four A-Priori Categories Guided Classification Of Usability
Issues. Inferential Analysis Compared User Group Performance And The
Two Versions Of The Interface.
Results: The 'Think Aloud' Data And Usability Observations Were
Combined To Create A Descriptive List Of Categorised Issues Which
Directed The Second Iteration Of The User Interface. Task Completion
And Task Duration Data Were Indicative Of The Operational Capability
Of The New Technology. The Pssuq Data Reflected Overall User
Satisfaction But Did Not Guide Identification Of Specific Usability Issues.
Inferential Analysis Quantified Comparison Between User Groups And
The Two Versions Of The User Interface.
Conclusion: Synchronous, Remote Usability Evaluation Is Feasible With
Pws And Represents A Solution To The Challenges Associated With Face
To Face Data Collection. The Co-Designed, User Evaluated Interface
