I highly encourage SIU Investigators to contact state fraud bureaus
to discuss referrals in greater detail as that added communication
can help expedite the allegation review process. SIU Investigators
should be aware however that confidentiality statutes often prohibit
state fraud bureaus from sharing any information regarding the
status of a referral or an investigation, so communication with a
state fraud bureau may be limited.

We thank each of our panelists for their forthright and informative
responses and we hope this will help you as you prepare your case
files, especially when considering forwarding the case to your
DOI. We invite you to contact your state's DOI. Get to know them
on a personal level and find out what they're looking for and how
your organization can help them pursue suspected criminals.
They're looking for cases and for help from SIUs. As mentioned,
any strong case is worthy of the DOI's attention. Working with
your DOI, together we can present a strong front in the battle
against those committing insurance fraud.
Armand Glick accepted the position as the Director of the Utah Insurance
Department Fraud Division in January of 2009. Prior to his appointment he
worked 21 years for the Salt Lake County Sheriff's Office where he retired as
a Captain. During his tenure with the Sheriff 's Office he served as Chief of
Police Services for several contract cities, including Herriman, Riverton, and
Bluffdale. Armand graduated from Weber State University with a B. S. Degree
in Criminal Justice and obtained a Master of Public Administration degree
from Brigham Young University. He and his wife live in Riverton Utah and are
the proud parents of four children.

It is our belief the ability for SIU Investigators to reach out to
our fraud personnel play an important role in the success of both
entities. We encourage the close working relationship between
SIU's and our sworn personnel regarding referrals and cases. It's
our belief the two must work together closely and communicate
frequently in order to effectively combat insurance fraud. As a
regular practice, fraud and SIU personnel attend intelligence
meetings in an effort to discuss specific referrals, cases and/
or trends impacting a certain type of fraud or region. The SIU
community is also provided contact information for all State
Fraud personnel and we make ourselves readily available to answer
questions or discuss referrals. We attribute much of our success
to the joint effort and close working relationships between State
Fraud and Insurance industry personnel.
Evangelina Brooks, Chief, Division of Investigative & Forensic Services,

Michelle Rafeld has served as Assistant Director of Fraud and Enforcement for
the Ohio Department of Insurance since 2007 and is responsible for overseeing
the investigation of unlicensed insurance activities, agent misconduct and
insurance fraud. Rafeld is a former insurance agent and has worked in the
insurance industry since 1990. As part of her duties, Rafeld actively represents
the Department on a number of committees formed by the National Association
of Insurance Commissioners (NAIC). Rafeld is often called upon to offer insight
regarding issues and trends related to agent misconduct and insurance fraud, and
currently serves on the Healthcare Fraud Prevention Partnership, the National
Insurance Crime Bureau's Law Enforcement Advisory Committee, and the
Executive Committee of the Coalition Against Insurance Fraud.
Michael Beavers serves as a Deputy Commissioner for the Virginia Bureau of
Insurance. Mike oversees the Agent Regulation Division and has been with the
Bureau for over 32 years. He has worked as an Investigator, Senior Investigator,
Investigations Supervisor, and Chief of Investigations for the Agent Regulation
Division. His current responsibilities include the operational oversight of
all three of the Bureau's investigation units and the agent licensing section.
Mike has also worked on legislative matters for the Bureau for over 24 years
and testified before numerous committees on agent related issues. Prior to his
employment with the State Corporation Commission, Mike was a full-time
private investigator with a firm in Richmond for five years. He holds a bachelor's
degree in administration of Justice from Virginia Commonwealth University.
Mike has earned the professional designations of Certified Fraud Examiner,
Chartered Property Casualty Underwriter, Certified Insurance Examiner, and
Certified Insurance Counselor.
Evangelina Brooks began her law enforcement career in 2002 with the Vero
Beach Police Department. She joined the Division of Investigative & Forensic
Services, Bureau of Insurance Fraud in May 2006 and was assigned to the Miami
Region specializing Personal Injury Protection (PIP) Fraud investigations. In
January 2009, she was promoted to Lieutenant of the newly formed West Palm
Beach PIP Squad. In 2014, she and her squad were filmed for A&E's American
Takedown, which went behind the scenes of some of America's biggest criminal
cases. The episode, White Collar Fraud followed the takedown of a large-scale
PIP fraud case, and allowed viewers an in-depth look at the inner workings of
complex financial crime investigations. In January 2015, she was promoted to
Captain of the Insurance Fraud North Central Region. In 2017, Evangelina was
promoted to Chief, overseeing the Bureau of Insurance Fraud which consists
of 10 field offices and approximately 140 members. Chief Brooks attained her
undergraduate degree from Bethune-Cookman College and her Master's Degree
from The University of Louisville. She is a graduate of the FBI National Academy,
Command Officers' Development Course at the Southern Police Institute and
is a Certified Insurance Fraud Investigator. She is also a FDLE/CJSTC certified
instructor, specializing in all areas of high liability.
