Help Needed With Oral Lesion
Help Needed Lesion
Introduction:
A 17-year-old patient presented to my office yesterday with chief complaint of "small painless bumps on my cheek and upper lip." She is currently on Z-Pak and Flovent from MD for
upper respiratory infection for two days. No other significant medical history. No other growths
or reactions on rest of body per patient. I referred her back to the MD treating her for the
upper-respiratory infection to evaluate possible reaction to meds. His diagnosis was canker sores.
Overnight she stated that the lesions became painful and started to grow.
She returned to my office today with some discomfort when pressing cheeks externally. The
pictures show that the lesions completely changed over the 24-hour period. I punted the case
to my favorite OMFS for evaluation and treatment as this is not something I have ever seen or

Defi nitely not canker sores. So the patient initially developed blood-fi lled bullae which
then ruptured and became ulcerated. The clinical features, along with the acute history, make
me think of an autoimmune condition. More specifically, I think erythema multiforme is a
possibility, which is known to be initiated by drugs, especially certain antibiotics. Are there
any skin lesions? Was there hemorrhagic lip-crusting? ■
