If treatment was pursued, she appears to be a Class III that is incompletely compensated. The
leveling of the upper arch appears to be reasonably stable with an appropriately flat occlusal plane.
The lower arch, however, requires a flat occlusal plane to match. The ceph shows mesial crown
tip of the lower posterior segments. If she still had 8s, extraction of lower 7s and substituting 8s
with distal uprighting of the posterior segments may be favorable, but that's out.
I would consider bonding segmental lower 7-3 and continuous upper Class III vertical elastics
until buccal segments are Class I and closed, then bond MD 2-2 apically and pick up. This is
to establish a flat mandibular occlusal plane. Essentially David's HSD, but reversed for Class III
and no spaces to close.
It would be nice to see a ceph from her initial pretreatment-something doesn't add up to
me. Thanks for posting. ■

He has spoken at the last two AAO and is in the Midwinter open bite meeting. I would
highly encourage all to read the series as they are published. Nice to have some data/etc. from
private practices.
Segmental ortho like the HSD is excellent for these type of Class IIIs. Just so many unknowns
in her case. ■
