The periodontist said he needs to get it straight so he can clean around it better. Patient fi nished comprehensive ortho treatment " in 7th grade " around age 13. Why did this 7 not drop down into occlusion? What are the chances it got ankylosed somehow? I told the patient there is a risk of ankylosis but I'm honestly not sure now how great the risk is. Plan is to use cross elastics and aligners to align the 7s and correct minor relapse misalignment. ■ 1/7/2020 davidharnick Member Since: 08/28/02 Post: 2 of 11 What a beautiful ortho result. I would conclude that this case was done with care and the UL7 just wouldn't cooperate. I would give this a poor prognosis. If you could design something that would not change the occlusion and existing tooth relationships, could be worth a try. It is not in contact with UL6 and he's 27 years old. Why did this happen? I'd guess the root was damaged when the 8s were extracted. He appears to have an excellent occlusion and aesthetics. What are aligners going to do for him? ■ 1/9/2020 emac Member Since: 11/21/11 Post: 4 of 11 A few choices I see. 1. Do nothing. 2. Surgical subluxation into occlusion, bonded wire from 6-7 for healing. 3. Band both 7s and run elastics buccal and lingual. The occlusion should keep lower in line and see if the upper extrudes. Probably won't erupt and you could add some surgery to try to loosen it up. I have had some success doing that. 4.
