TEACHING CERTIFICATED PILOTS.
Two-thirds of all fatal accidents during fixedwing instruction came on flights where the
"student" already held a pilot certificate. Even
more surprising, only 40 percent of the accidents
during advanced training occurred while
working toward some higher certificate or rating.
Pursuit of logbook endorsements (complex,
high-performance, or tailwheel) accounted for
a few, but the greatest share took place either
during recurrent training-flight reviews,
IPCs, or unspecified refresher flights-or while
transitioning clients to unfamiliar aircraft.

Veteran CFIs seem to have no difficulty envisioning the dynamics involved. Instructors asked to
provide transition training don't always have significant expertise in that specific make and model.
Instead they (and their students) rely on their presumed ability to monitor the flight, recognize
potential hazards, and intervene in time to avert catastrophe. When the aircraft's not just unfamiliar
but unusual, this can be a bad bet. Likewise, CFIs giving flight reviews or IPCs in aircraft owned by
their students would like to believe those students know their own machines well enough to avoid
dangerous situations-while the owners trust their CFIs to keep them out of trouble. Functional
responsibility for the safety of the flight can slip between the cracks.
Case in point: an ill-fated flight review at the Livingston County, Michigan Airport on June 29, 2011.
The airplane was an amateur-built Thorp T-18 that had received its first airworthiness certificate
nearly 30 years earlier, then been recertified five years later with a different engine-propeller
combination. Its current owner, a 43-year-old private pilot, had operated it for about a year and a
half, but not frequently. His logbook showed 43.4 hours in the T-18 and 166.3 hours of total flight
experience. He'd logged no flights since the previous November, but the NTSB noted that the
airplane's Hobbs meter "suggested that the pilot could have flown as many as 6.6 hours in the accident
