A Competency examination Report form that was provided with the competency exam letter presented to the examiner. Note: If the customer is under 18 years of age, the license application must be signed by a parent, guardian, child guardian division, or boarding school headmaster. If the person signing the application is not a parent, proper documentation must be shown at the time of the road test to prove that the person is qualified to sign. 4. A completed Vision Screening Certificate if the customer chooses to submit a certified rater than have a vision test performed during the test. Results of a Competency Road Test Results of the competency road test are sent to the Medial Affairs, also with a statement indicating whether any infraction or motor vehicle laws occurred during the course of the competency road test. Any customer who is aggrieved by a Registry of Motor Vehicles decision has the right to bring his/her case to the Board of Appeals. Active License - If a customer with an active license passes the competency road test, then the license remains valid. Failure of a competency road test may affect the customer's license status. Suspended or Revoked License - If a customer with a suspended or revoked license passes the competency road test, the license will be reinstated. If the customer fails the competency road test and is not cleared for retest, the Medical Affairs will place a medical hold on the license. If the customer fails the competency road test but is cleared for retest the Medical Affairs will re-send the competency test paperwork to the customer. 12/16 13:31
