Chapter 9-4 Ignition Interlock Devices (IID) Installation/Transfer You must first visit a RMV branch office and speak with an RMV Hearings Officer. The RMV Hearings Officer will explain the criteria for a conditional hardship license or for full reinstatement. DO NOT have an IID installed in your vehicle before you have received approval for the IID from an RMV Hearings Officer. Please refer to the Hearing Sites and Schedules to find an RMV branch office that holds hearings. Once you have been approved for a conditional hardship license or for full reinstatement, the RMV Hearings Officer will provide you with a list of approved IID vendors/ service providers. You will also be given paperwork and legal documents (affidavits) that must be completed, signed, notarized and returned to the RMV Hearings Officer. By signing these affidavits, you certify that you understand, as well as each licensed driver in your household, that you are not allowed to drive any vehicle without an IID and that you cannot try to bypass the IID in any way. The penalties for these actions are included in the affidavits. Steps for Installation: * Step 1 - Choose a certified IID Vendor and schedule an installation appointment * Step 2 - Complete the Ignition Interlock Operator's Affidavit that certifies that you understand that you are not allowed to drive any vehicle without an IID. The Ignition Interlock Operator's Affidavit must be notarized. * Step 3 - Complete an Ignition Interlock Affidavit - Additional Licensed Resident for each licensed member of your household. 06/16 9:66http://www.massrmv.com/Portals/30/docs/interlock_operator_affidavit_01061.pdf http://www.massrmv.com/Portals/30/docs/interlock_additional_resident_info_affidavit.pdf
