Chapter 4-12 Passenger Plates - Lottery The notarized application must be signed under the penalties of perjury and before a notary public. Each affidavit must describe the nature and length of the significant personal relationship between the Transferrer and the Transferee. 2. If the holder of the plate is deceased, the notarized statement requesting the transfer of the plate to either an immediate family member or spouse or to a person with whom the deceased has a "significant personal relationship" must be prepared by the Executor or Administrator of the Estate. If the Executor or Administrator of the Estate is either an immediate family member or a person with whom the deceased has a "significant personal relationship," he/she can transfer the Lottery plate to him/herself. A copy of the Notice of Appointment from the Probate Court identifying the executor, administrator or other personal representative of the decedent's estate must be shown. No transfer can be made from the deceased person if no Executor or Administrator has been approved. The registration can remain valid until expiration but cannot be renewed. A copy of the Probate papers naming the Executor or the Administrator must be submitted to the RMV along with a completed notarized Application for Lottery Plate Re-Assignment. 3. Re-Assignments to a leased vehicle are permitted as long as the leased vehicle is in the custody of the holder of the plate. The leasing company must return the plate to the original plate holder upon termination of the lease or at the holder's request.
